Film about the girlfriend of a heart donor

Do you know this film? A girl whose dead boyfriend's heart has been transplanted, tries to find the transplant recipient. She arranges a meeting but the recipient turns out to be a fat, middle-aged man. She stays hidden, but follows him for a while, then eventually steps off a tram/streetcar and walks off. The film might be Italian.
...I saw this on SBS TV in Australia many years ago, and have been looking for it...but none of the film buffs I know, know anything about it. It's very tragic, as the woman seeks to be with the one part of her boyfriend that is still alive.

There's a page on IMDB for films about heart transplants. Maybe one of these?
I initially thought Heart Condition was it, but maybe it's just Heart.

Is it possible what you're talking about is Almodovar's Todo Sobre Mi Madre? It's the same idea, only it's a mother and her son's heart. Other than that, it fits.

Response by poster: Thanks to all, but it isn't "all of me", nor any of the "heart" related movies on IMDB. Nor is it Todo Sobre Mi Madre.
It was a short film, about 10-20 minutes. But thanks to all who responded...I'll continue my quest!

Apparently, there's a story called "All of Me" that sounds something like that. Described here and here.
