How should ADA paperwork take in a large corporation?
(Company name purposely withheld for obvious reasons). It's been 3 weeks since I dropped it off. I'm going to query HR today.

I've been an employment lawyer for over 15 years and your question is confusing to me. There is no universal "ADA paperwork." I'm not clear what your paperwork is for -- a requested accommodation? A leave of absence? Are you sure you're not referring to the FMLA, which does have designated paperwork?

I am not referring to the FMLA, although I know what that is. This is a special form ("Request for Consideration of Accommodation" or some such) that [Large Corporation] has made up; it requires a physician's signature. Perhaps their lawyers wrote it, I don't know. They send it to Corporate, in another state, and then their little committee decides if your request for an accommodation is valid or not.
Sorry if that wasn't clear.
