Books/lectures about healthcare?

Where can I find a good book (or audiobook) explaining the history of healthcare, and the current state of affairs?

I'm tired of not fully understanding how we got where we are (short of vaguely knowing that the Johnson administration started Medicare/Medicaid) with the state of healthcare in the US.

Moreover, I'd like to better understand other healthcare systems around the world (e.g. Germany's Bismarck system or the UK's NHS).

Podcasts are ok, but I prefer something a bit longer than a single episode of This American Life or something, since I really want to understand things.

So, what books/audiobooks/lectures/podcasts can you recommend?

You're probably looking for something much meatier, but This American Life's two episodes on healthcare: More is Less and Someone Else's Money is a good appetizer.

Haven't read this but have heard glowing reviews: The Healing of America: A Global Quest for Better, Cheaper, and Fairer Health Care by T.R. Reid, who was a Washington Post reporter & made some Frontline episodes about health care.
