I need a doctor in brooklyn

[BrooklynFilter] I need a good GP, preferably in south Brooklyn, who takes GHI.
I live three stops up from Coney Island, so the "south" in this case is "way the hell down south." Awesome if they are accessible off the D, F, N, B6 or B82.
A few caveats: Under midtown in Manhattan is okay. North Brooklyn (Williamsburg/Bushwick/Greenpoint) is right out; it takes me longer to get there than it does to Manhattan. Queens or the Bronx? Not even close.
Otherwise, what it says on the tin.
