I use that plug-in on my blog. You enter the text in the image to the right in into the box.

Have you looked at the site of the people who wrote the plugin (dd-formmailer)?

(Is that what you're asking about? I'm kind of confused...)

It appears to be tied to the IP address. The server probably just remembers the last verification code sent to a particular IP address (subject to timeout, etc.) Open two copies of the page. Enter the code from the second one you opened into the first one...

do you have flash or adblocking software on? that could be impeding the verification code.

That image looks absolutely *tiny* (unreadably so) under Safari. You can get at the image by right-clicking (or ctrl-clicking) on the image and choosing "open image in new window" from the menu.

Thanks for the suggestions, still no luck. I get "No verification code generated" with every string of letters/numbers I copy from the image, which I can only see doing as strangecargo suggests (I'd gotten that far before posting here), regardless of how many times I reopen the page or reopen the image. Ad blocking is off in Safari.

Doesn't work for me in Safari either. I'm too lazy to figure out why, and presumably so were the developers of the site.

Works in Firefox though, so if you really need to use that form, try a different browser.
