Personal year in review

Several years ago, I was linked to website offering a pdf that you could fill out as a personal year-in-review. I remember that the landing page was fairly spartan and had options to print it in different formats (letter, A1). Inside, I remember that one of the questions asked you to look back at your calendar for the year and pick out memorable things that happened. It was meant to be done either solo or with a partner. I lost the bookmark, and can't seem to find the right incantation to throw at google.
