Customizable coaching app?

I'm looking for a highly customizable app or tool that functions like a flow chart. I use OSX and iOS.
This app would throw up a prompt; receive a yes, no, or other defined input; and move on to the relevant next prompt. Ideally it would be able to loop.
I have no programming skills.
Bells and whistles might include alerts and ability to link to URLs and documents, but that's just icing on top.

Response by poster: I've had a chance to make a couple of "stories" in Twine, and it's just about perfect for what I want. Now I just need to figure out a way to get it to my iPhone in a way that keeps it private. Thanks so much.

Best answer: I'm glad it worked for you! It looks like if you do the following, you can keep it private and open it on your phone:
1. Press the settings gear on the story and "Publish to File" (this just saves it as a file to your computer's hard drive -- it doesn't publish it publicly).
2. Save that file in a place that allows you to access it on your phone -- for instance, to Google Drive, Dropbox, or iCloud. You could even email it to yourself if you don't have one of those services set up.
3. From your phone, download the file and then open it with your phone's browser.

Response by poster: I had tried opening a file sent via email, but that didn't work for whatever reason, and I assumed my phone wouldn't be able to open an html file, period. I didn't actually download it first, so I'll figure out how to do that and give it another try.

Response by poster: Marking this as resolved, and following up for anybody who will find it helpful: I was able to open the dropboxed file in Safari on iPhone by changing the "www" to "dl" and then bookmarking that.
Twine was the perfect solution, thanks!
