Are there organisations in the UK to help people escape hate groups?

EXIT-Germany did a famous stunt that turned a Neo-Nazi march into a fundraiser for their organisation, which helps Neo-Nazis get out of that life. Are there groups in the United Kingdom who can help people move on from hate groups? What about the US?

I've been trying to change my online engagement style to be less smug and showboaty, and more generally constructive and sympathetic. This can be hard when the other person spews a stream of well-rehearsed hate.
I'd like to be more constructive even here, so being able to turn a discussion around with "listen, I don't know what your life has been like but there are groups in your area who can talk to you and help you get out of the blackshirts..." appeals to me. It may just be a rhetorical manoeuvre most of the time, but since there's a chance it could get help to someone who needs it I'd like to give it a try.
The problem is, I don't know where to find a reputable group in the UK or the US. I want to make sure I don't feed angry and lonely people into different dangerous cults.
Are there hotlines that I can hand out when engaging with people who need opportunities to reform?

You could try Hope not Hate - I think that comes under their remit, and if not they should be able to point you in the right direction.

Response by poster: Thanks for the tip, doop, but I look on their site and see this Round-up of militant jihadism in the news and worry that they might have their own problematic perspective.
