Name of novel where US is evacuated, Europe filled w/ American refugees

There's a novel I remember getting from the library when I was very young, and it mostly flew over my head. I'm trying to remember its name. I believe the premise was the US had to be evacuated for some reason, and much of the rest of the world filled up with refugee camps and ghettos of Americans.

The cover was possibly blueish, possibly with cities under domes in a desert-ified landscape.

I recall very vividly a scene where Congress has its final session, and someone bangs a gavel which either eliminates the US or creates a government-in-exile. Something like that.

This periodically bugs me, and Google has failed me.

I'm not sure but it might be Hello America by J.G. Ballard.

It has been a very long time since I read it.
