What is this piece of music?

I need to identify a piece of music based on this tiny portion of sheet music. I entered it into MuseScore and created a wav file (listen here), but SoundHound and Shazam don't return any results.
There are a few problems with the transcription, though: 1) I'm completely guessing on the 4th measure; 2) the bass clef for measures 4-6 are a guess based on the Ab, Adim7, Eb7, and Bdim7 notations; and 3) I'm almost positive I mixed up a couple flats/sharps/neutrals.
This is homework, but I promise it isn't cheating. This is the first assignment for a research class, and we are supposed to use every resource available to us to answer the questions, and then verify our findings with other sources. I'm also reaching out to a music librarian and a few record store owners, but I would greatly appreciate the help of the hivemind in tracking this down.
