iPad/digital guestbook for wedding?

My wedding is this week. I'm terrible with names. I am looking for some way to have a guestbook that allows pictures of guests (or even audio and/or video) along with their names and addresses (and any other message).

Something like an iPad seems perfectly suited to this task, and I could probably borrow one for the day, but does anybody know if there is a good way to do what I'm trying to do? I tried googling "ipad guestbook" and "digital guestbook" and it looks like there are a few apps out there, but they seem to have pretty mixed reviews. (Has anybody used any of them?) I'm also concerned that some of them might not let you easily export the data/pictures/etc. I obviously don't want all of that info to be trapped in some app that will probably go defunct in a few years (especially if it's not my iPad to begin with).

I'm also open to any non-iPad ideas. I don't think I have access to any other tablets. A smartphone just seems too small to have everybody trying to enter their addresses and stuff on (considering some guests may be older or not tech-savvy or whatever).

Any experience with anything like this? Ideas?

You want the iPad app called Wedding Booth - it does exactly that, I used it for my wedding. Takes a shot, then gives a prompt for guest to write a message with finger or stylus. Provide a stylus - much better!
