I need help remembering a book title for some serial killer fiction from the 1980's
What I remember is a teenager kills his mother, gets sent to a mental hospital where he escapes. He starts dressing like a woman to get close to women and then he kills them.
I realize it's not exactly Dickens, but I'd like to remember the title of the book.

That's what it sounds like to me too. Also, check out the 1990 movie based on the book starring Jodie Foster and Anthony Hopkins. A suspense classic!
