Online permission forms?

Online Forms: Help me figure out how to move my event specific permission forms and yearly medical forms online?
I'm desperately trying to avoid paper, and not just for the sake of being green. I work with youth, and frequently take them on trips (with other adults) that require them to have a permission form that is tied to a yearly medical form. I've refined both forms down over the past 2 years into what I need. However, each trip is a headache as I try to get the forms filled out. I would LOVE a way to do this online.
I know there are various ways to create forms... but what is the best way to have them "signed" online? I don't want youth forging the forms (though I'm not super-concerned about this).
I've recently moved to PDF's of my documents, rather than handing them out. So all of our documents are in a single online location which has been helpful. But I think if I moved to an online form, I'd have a much better chance of getting them before the actual event.
What legal concerns might I have with this option in the event that I would go to court?
As a side note, I'm also trying to switch to online even registration and payment, just setup my first EventBrite event. But if there was an alternative, that would somehow integrate with a permission form and parental consent, I'd use that in a heart beat.
