Where can I find a sample agreement for an affiliate program?

I want to set up an affiliate program for a product I am launching soon. I've got all the technology I need for it, but I need to have an agreement page where the affiliates agree to a number of things (not referring themselves, spamming sites, etc.).

Where can I find text that says all the things affiliates should be aware of? Sample text would be OK, but I'm really looking for something I can basically lift and use with minor adjustments.

You could sign up as an Amazon affiliate or similar, then use their terms as a basis. Or try this search but I would suggest this would be a reasonable little job for a lawyer in your jurisdiction.
Otherwise, if somebody violates the agreement, you are unlikely to have a strong legal basis to act against them. I can't imagine it would cost more than $100-$200 to draft an enforceable agreement if it is truly generic. IANAL

Sign up for a few - Linkshare, any of the Digital River sites (eSellerate, element5, RegNow), Commission Junction, Amazon, etc. - and lift the parts you like out of each one.
