Uzumaki and various short stories (Long Dream, The Enigma of Amigara Fault, The Window Next Door, etc) by Junji Ito.

I came in here to recommend Tomie, by...Junji Ito. And Hell Baby by Hideshi Hino.

Stuff by Shintaro Kago, he lies somewhere on the guro/horror spectrum.

Suehiro Maruo's The Laughing Vampire. It's on the guro side as well but his artwork is just incredible.

If we're expanding this a little to include things that edge on guro, Tsutomu Nihei's work (Biomega, BLAME, and my favorite, Knights of Sidonia) deserve inclusion.

Also -

Dorohedoro: a man with the head of a crocodile, magic that mostly seems to twist and pervert its users, and a perfectly sketchy art style.

Devilman: Go Nagai don't give a fuck.

Kazuo Umezu's Drifting Classroom. Very intense and horrifying on a couple of different levels.
