Need recommendation for very good PMP classes in the NY/NJ area? My company will foot the bill. Please outline why you think the classes that you are recommending are good. Thanks in advance

Also are any sample filled out PMP application forms accessible via the net?

I dont want to copy just want to get an idea. I believe filling out the form can be a challenge.
