At work we use Bandwidth.com for our VOIP carrier. We've been using them for about two years now, and haven't had any major issues that I can recall. We have around 25 employees on four continents, and our call quality is generally crystal clear. When we have had issues, the guys at Bandwidth.com have been pretty quick to address and resolve them.

voip.ms works well for me - haven't used them on a large scale, but their no-nonsense approach seems really easy to work with.

Are you sure it is the VOIP service and not your internet connection?

Anecdotal, but I've heard excellent things about Simplesignal.
