Objection! Overruled! Does this actually happen like in tv shows?

Seeking input from either lawyers or police. How accurate are crime/police shows? Details after the break.
Some of the things I wonder about are:
* Is it actually common for a suspect to breakdown and say they did it in questioning?
* Why would someone not just say they want a lawyer first thing? Or is that common now? Yeah, it makes you look suspicious, but you are already a suspect.
* Does it ever actually happen where a witness blurts out a confession or implicates themselves in a crime during a trial?
* Is it frequent to see rapid fire "Question" "Objection" "Withdrawn" exchanges that are frequent in court dramas?
Even if I were to know these things don't happen for real, it will still be fun to watch, but I am curious about how much could be real and how much is fiction.
