Who can forge this ancient artifact once more?

Are there any reliable companies/artisans who can create custom body jewelry? Specifically, one who can recreate this now out-of-production Stinger Talon?
I have one of these in 4g that I've used for something like 15+ years, but the pierce hole itself is closer to 0g and the talon falls out constantly these days. Unfortunately, it doesn't seem like anyone makes it anymore, and I haven't had luck finding a place that makes custom pieces (outside of prince's wands) that are more involved than 'pick what gem to put in a captive ball'. There are lots of labret items closer to this still around, but that teensy post won't work at all.
Someone has to make claws like this - does anyone have experience with a place that does custom one-offs?
