Templates for printing pages to track elderly parent's health?

My mother is already writing notes about her daily health actions. I'd like to improve the notes and add more information. Are there ready-made templates somewhere for printing and putting in a 3-ring binder?

Health journals exist but they are usually wire bound (which means a limited number of pages) and/or overly complicated. I could use some ideas for starting points to create something to put in a 3-ring binder. Surely there must be printable templates available somewhere?

I'd start with the worksheets available at careforagingparents.com. I use these for my mother and they're a good place to begin to keep track of things.

Would something like this fit the bill? I think you wouldn't necessarily use all of the pages included, but you could pick the ones that look good (maybe the weekly wellness one that looks like a calendar) and print out as many as you needed.
