Buying Street Drugs on The Streets of The USA

I am- regrettably- a long time heroin user going back around 15 years, living g out most of that time here in Sydney, Australia. In all that time I have had a number of suppliers, generally referred by friends of friends. In all my long and storied career not once have I picked up from random dealers spruiking on the street-it has always been a case of making phone calls, meeting at an arranged spot and transacting the deal accordingly. .
However... If what I've read and seen on the TV and so forth about the drug scene of the USA (for example the various works of David Simon) deals literally happen on street corners between random strangers in broad daylight, without introductions, with a level of organisation like small businesses! Is this really the case? Is it really that easy to acquire substances? How much quality control could there be? What sort of price do people pay for heroin? Is it really so rampant and blatant as I am lead to believe? Are there needle exchange programs/ is clean injecting equipment easily accessible/ accessible at all?
Can you point me to any resources, articles about pricing and the street drug trade in the USA? Thanks very much- I know this is a sensitive topic for a lot of people- for me in particular it is something that effects my life on a daily basis.
