Japanese translation help

Help me translate this tea cup from Japan...

My friend gave me this tea cup he got when he was living in Japan as a teenager. He no longer remembers what it says. After converting the image to characters google gave me this:

It says Take-sushi, which would be the name of the sushi shop, and has its address.

Ootemon chushajou-dori
Denwa (Phone) 32-2454

Like Metro Gnome says, it's the name, address and phone number of a sushi restaurant, probably in Matsumoto, Nagano Prefecture.
