Elder Law Question

Need some help with elder law. This is in Pennsylvania. I need a template, word doc or fillable .pdf for Durable Power of Attorney for Finance and Health Care.
I'm having a hard time sorting through Google search results, many lead to pay sites and it's my understanding that the language is pretty straightforward and has been vetted and I really don't need an attorney to prepare the document. She has a living will, that's covered. My mom fell, broke her hip and now is in rehab center and they are kind of sweating me for this so this is time sensitive. Thanks.

Sometimes states have a statute that is the standard-form language for a living wil / durable power of attorney. Connecticut does, and (IIRC) Illinois does.

valkyryn: That's the health care portion (aka living will) which we have in place. My understanding (and what the care facility has explained to me) is that this is not sufficient.

FWIW, valkyrn's linked document looks a lot clearer than the attorney-prepared one my mom gave to me a few years back (my mom lives in PA).
