I’m looking for memoirs in which a patient has sex/an affair/a relationship with their doctor. I’m tentatively open to novels, with the caveat that they have to be realistic. I don’t want a romance novel or something that’s a fantasy about how someone would have liked it to happen, I want something realistic that shows how that sort of situation could actually play out in real life.

I don't think this is really what you are looking for but in the Ian McKewan novel Saturday, one of the main characters is a neurosurgeon whose wife is a former patient he operated on.

This may be completely wrong because I don’t have a specific book, and it’s well over a century ago now, but Dr. Samuel-Jean Pozzi was a preeminent gynecologist who was known to have had affairs with many of his patients, most notably the actress Sarah Bernhardt. If it is within what you’re looking for, I’m guessing there are books out there (I learned what I knew from the book Strapless, about John Singer Sergeant.)
