Insulting / racist comments at work - hypothetical scenario

Can a coworker get you fired solely on his word with no proof or evidence of what he is accusing you of?
Okay, this is just a hypothetical scenario. Say that you are in the bathroom and you mutter something to yourself something offensive or racist, and then a coworker happens to walk in the bathroom and heard what you said. There is no one else around, just you and the coworker. What happens if that coworker decides to report it. Is it just a case of your word against his? Or will a coworker's description of the incident be enough to get you fired? (What if you said a racial slur not related to that coworker's race?)
And then scenario 2 will be what will happen if a rumor spreads about this bathroom incident to everyone else in the company? Will a rumor be enough to be you fired?
In other words, for any type of insult or incident, can someone's word be enough to get your fired. Or does the company actually need proof or hard evidence to let you go?
