iPod drifting out of sync: why?

I have no idea whether everyone with an ipod has this issue or if it's just me doing something wrong: the track listing on my ipod in iTunes on my computer is way out of sync with the ipod's real contents.

It's an ipod touch about 2 years old, works fine except for this. I add and remove mp3s fairly often, listen to an audiobook or whatever and delete it. Right now the listing on iTunes is full of old files I deleted ages ago. Syncing the thing does not fix this. The only thing that does fix it is doing a full ipod restore, after which the listing will be accurate at first then gradually drift out of sync until it's useless again. A full restore is a pain so if I can fix this without doing one, I will.

Should note that the ipod's own music player lists the true contents. It's only when I look at it via iTunes on my Mac that it's all wrong, but I'd like to see the real contents in iTunes as well.

What operating system is on the iPod? What version of iTunes are you using?

Response by poster: iPod: version 6.1.6, the latest that will run on the device
Mac: Mini running Yosemite and iTunes 12.1

The problem has been occurring for on the order of two years, though. Several updates ago on both devices.

You're not alone. It's a long standing problem. This helped me fix it, (See the part on duplicates) but I expect that it'll happen again.
