Looking for a shirt like the one in this pic
The cuffs are the major part - I've been searching all over for "ruffle cuff shirt" and the like but nothing is really popping up that is similar (in mens styles, at least). Preferably in purple or red, but other colors may work.

I have a red vintage tux shirt with ruffled cuffs. Look for vintage tuxes.

Also, being a staple of just about any goth scene, check where your local goths shop.

Looks like the sleeves of my ruffle tuxedo I wore as part of the Young Catholic Musicians (in the 90s...the tuxes were from the 70s).

Yeah, I have a repro tux shirt with those kinds of cuffs. It's off white/cream. I also have a class maroon tuxedo jacket to go with it!
