Name this heroin and breast-implant story?

What's this novel (featuring a heroin dealer and a bad boob job)?

I remember reading as a teenager (which would make this the mid-90s) a novel excerpt in a magazine (GQ or the like) which starred a heroin dealer on (I think) the streets of San Francisco who kept balloons of heroin in his mouth. There was a subplot about a stripper who had gotten a horrific boob job from a bad doctor who used some type of auto-mechanic tools to perform the operation. It's possible that it was just a free-standing short story, but I'm pretty sure it was the first chapter of a novel.

Best answer: It's probably Homeboy, by Seth Morgan, and it's a wonderful, wonderful book. The dealer is Joe Speaker and the stripper is Bermuda Schwartze. Sadly out of print (I think) but easily findable, I would imagine.
