I need a custimized webform.

Need to create a webform with responses and .jpg attachments that go stright to email. The key word here is easy.

I have almost zero coding experience, save for the VB coding that I did several years ago in high school.

My company has technicians who install large robotic machines late at night. I have been asked to create a "post-install checklist", but feel that it would be beneficial to make it a webform because the robotic machines we install do have a web browser on them, meaning that no one has any excuse not to turn one in after an installation.

In addition, each technician must take 3 photographs of each install and email them in. Anybody have some good advice on creating a webform that meets the following criteria:

*Unlimited customizeable fields
*Email output to predefined address(es) (I can provide pop/smtp servers)
*File attachemts to said email (ok, or links to an online host)
*E-A-S-Y

Best answer: Check out Wufoo. You'll need to pay to use file upload, but pricing starts at $9 per month. Totally awesome way to build web forms.

I was coming here from my RSS reader to suggest wufoo as well. Zero coding needed, you just point and click what you want in the form and it builds it automatically.
