Getting a book published

What steps to go through to get a book published?

A friend of mine has severe arthritis but remains a body builder. Someone at Arthiritis Foundation wants him to start giving inspirational speeches and suggested he write a book about his experiences.

How would one go about getting the ball rolling on a project like this? Since this is a 'nichy' book, would you still need an agent? Would you go to a publisher first? How would you go about finding a ghost writer? Anything else I'm not thinking of?

I think he should ask this person if the Arthritis Society can publish it. They're the ones who can best reach one of the key target audiences.
