Design of web-based learning tools

I’m looking for examples of web-based learning tools, specifically, self-testing tools such as flashcards, multiple choice, and fill-in-the-blank.
I’m primarily concerned with design and usability aspects of these tools, rather than their function or domain. I’d like examples that you’d consider well-designed and very usable, as well as examples of poor design.

I have used QUIA for years, and am very impressed with the design and usability. I work with teachers at the high school level, and even the most computer illiterate of them can always figure out how to use the tools on the site. They offer (I think) a 30 day trial if you want to create your own.

As a homeschooling Mom, I often use AAA Math for review. It's not fancy, but it gets the job done. I have used it myself for a quick lesson on the math topics I never really understood as a child.
