How to declare drug import for personal use

I use a medication that is prescription in the US but OTC in Sweden and at much lower cost. A lovely friend is willing to send it to me. FDA guidance makes clear that importation for personal use is not an enforcement priority. What is the best way to declare this on the customs form?

The FDA internal guidance says:

In allowing personal shipments of drugs or devices, FDA personnel may consider a more permissive decision in the following situations:
1. When the intended use is appropriately identified, such use is not for treatment of a serious condition, and the product is not known to represent a significant health risk;


Should we put on the form “ OTC drug for personal use, 3-month supply, not for treatment of serious condition, no known significant health risk”? Or will that unnecessarily invite scrutiny? Google is not providing answers on this one so any personal experience with this would be helpful. Thanks!

I wouldn't draw any attention to it being a prescription. I recently ordered an rx through Canada and it was confiscated by the FDA. No risks, no addiction concerns, available in the US. $542 USD, $119 CDN. Though the FDA was professional and empathic, my mail was denied.

When I've had things like this sent to me, they've been labeled nutritional supplements/vitamins or cosmetics.
