Please help me find the perfect protein shaker-bottle.

I am looking for the perfect bottle / cup / jar for carrying protein shakes. Pickiness inside.
This perfect shaker bottle will: be COMPLETELY and EASILY washed in the dishwasher, daily, every single part, without it being a pain to disassemble.
Be insulated enough to keep a cold drink cold, in a hot car, for several hours.
Have a lid that is really leak-proof so it can be shaken with abandon.
Be wide enough to get one of those shaker balls in and that protein powder doesn't spill onto my kitchen counter, but narrow enough that it's not a mess to drink from.
Be available for sale in the US.
Nice but not essential: interior measurement markings, a rounded interior bottom so powder can't hide.

Best answer: I have not gotten an insulated bottle from blenderbottle.com, but I like their twist-cap shaker bottles a lot. Two pieces, no trouble in the washing machine, and never had a problem with leaking except when I assume my wife has actually closed the cap

The Strada Twist - posting the link to save other folks search time - doesn't seem all that expensive if you consider how well it's going to meet your criteria.
