Can you recommend any resources on survey design and other aspects of survey research?

Depending on how rigorous this survey needs to be, Sage Research Methods is the standard academic resource. You'd likely need access to a local university library.

If you want something 'quick and dirty', browse the Qualtrics support pages. If you're not familiar, Qualtrics is an online survey-maker platform (among other things). They have excellent technical help pages for how to use Qualtrics, but they also have some basic info on survey design. Their reporting and analysis tools are getting increasingly sophisticated as well, so they have some info on data analysis too. I believe they offer a free limited account, but you don't need an account to browse the help pages.
