Survey tool for collecting feedback/email subscriptions

I want to create a survey for existing and new players about a potential video game to embed in my website. What service/tool/plugin should I look at?

Requirements:
  • The website is Wordpress. I need to be able to embed the survey in a Wordpress page.
  • The survey will double as a mailing list sign-up form for Mailchimp, but it's okay if it isn't real time. I.e., I'm fine with exporting the list and importing into Mailchimp. Even if the data is just stored in a mysql table, I can figure out how to export it, so long as I have access.
  • Paid is okay, but I don't want to spend hundreds of $ every year forever just to have a single survey/form, which seems to be the business model for most commercial survey solutions.
  • The questions are a mix of free form responses as well as wishlist items. It would be ideal if players could do some kind of ranked voting, but a simple numeric scale would get the job done.
  • I haven't finished designing the questions, but I expect a minimum of 8 and a maximum of 12 questions.
  • Most of the questions will be optional
Feel free to offer any suggestions on web survey design in general, but my primary question is about finding the right tool.

Best answer: Check out Google Forms; you can see entries as a spreadsheet. The forms can be embedded.
