Best dining chair cushions?

Please recommend your favorite dining chair pads/cushions. Tiebacks are strongly preferred. Something that is comfy and won't flatten after a month. Price is not an issue, but I'm not exactly looking to drop 1K on chair pads.

My mother had four chair pads from the Vermont Country Store that held up very well to heavy use. I'm no foam savant, but they seem to be filled with closed-cell foam; the pads were relatively heavy and retained their loft for years. One or two of the ties tore off (I mended them) and the pads were ultimately scapped for an unrelated issue (they smelled like cigarette smoke).
