Finding new healthcare in NYC

Next year, I will be relocating from the SF Bay Area peninsula (Sunnyvale/Mountain View/Santa Clara) to New York City. I need to find healthcare providers for chronic conditions.

I have a constellation of chronic health conditions, which means I see a lot of doctors on a frequent basis, and have been getting most of my healthcare through Palo Alto Medical Foundation (PAMF). I want to find the same kind of one-stop shop in NYC.

I will almost certainly be living in Brooklyn, and will almost certainly NOT have a car, so I need to find providers who are Metro-accessible.

My insurance will remain the same, a "gold-plated" PPO plan that allows me to see anyone I choose, within reason. That is, no HMO-style restrictions on referrals and the like.

NYC-area Mefites, especially those with chronic healthcare issues, I would be hugely grateful if you could weigh in with your recommendations (for or against) for medical professionals, groups, and so forth, particularly in the following specialties:
  • Endocrinology
  • Nephrology
  • Cardiology
  • Psychiatry

If you get Emblem Health insurance, you might be able to see Montefiore doctors. It's a large network and they have lots of affiliated doctors and specialties.

Sorry, I realize you already have insurance. Well, check out Montefiore doctors anyway. You can see a lot of specialists through them.

I tend to use doctors affiliated with NYU, and have been happy with them.
