Free feedback form

I want to add a FREE very simple (name, email and text area only will do) feedback form on a Blogger blog to let site users send feedback, suggest links etc.. Which is the best web service?

Form-Mail.com seems to be offering this service for free, with limitations. formmail.com seems to be a completely different outfit which has a similar service, though it's not clear if it's free.

SiteGadgets' Complements script sounds kind of close to what you want, and it appears to be free as well.

The CGISpy Feedback form also appears to do what you want, and is hosted for free.

I found all of these with just a couple of quick Google searches:
Searching for cgi host mail
and searching for form mail

But I have no opinion on quality of any of the above. When I want to run scripts, I put the scripts on my server and run them.
