How does my notary complete the bottom portion of this form?

I am currently working on obtaining an official notarized copy of an Affidavit of Single Status. The bottom section of the form is a bit confusing in terms of what the notary is supposed to fill in the blank spaces. (A link to the Word document in question is included in the explanation.)

I am currently working on obtaining an official notarized copy of an Affidavit of Single Status (sometimes called a Declaration of Non Impediment). I have completed all the necessary information in the top section. My notary has completed everything except the bottom portion of this form:

My notary is well-versed in any number of documents and forms, but has not seen one like this. It requires several pieces of information at the bottom of the form (intended to be completed by the notary public). The first line is obviously supposed to feature the date. What is supposed to go in the blank spaces on the second and third lines of this section?

If it helps, this form comes from a notary/apostile website that has several forms of this nature.

Best answer: The line after "personally appeared" should contain your name. The next line after "which were" should list the ID you gave the notary (e.g. driver's license, passport, etc.).
