Info about Nan Goldin photograph

I have this Phaidon Nan Goldin monograph written by Guido Costsa, and there isn't any info or even title/date of the (NSFW) cover photo. It is the same photo as you see here: link. I would like any info available about this photo. Does anybody know who the model is, what her relationship was to Nan, when the picture was taken? Any other interesting info? Thank you.

My suggestion would be to contact her gallery, Matthew Marks, and ask. She may also be on Facebook and Twitter; at least there are accounts with her name.

Best answer: Google Image Search tells me that it's called "Joana's Back in the Doorway". If you search for "Nan Goldin Joana", you find more photos of her.
