Is there any really (genuinely) weird hiphop/rap/R&B-ish stuff out there a la Tricky's Pre-Millennium Tension?

Depends on how weird you want it, or what kind of weird. Dr. Octagon seconded (though you should avoid the "sequel" like the plague). Gonjasufi's album on Warp earlier this year might be up your alley as well- here's a representative track and its B-side from the single release.

Anything Kool Keith has done falls in this category, not just Dr. Octagonecologyst. Another whacked-out album is Nogatco Rd.

If you're looking comparably claustrophobic, tense stuff, but with a much more raucous feel, you might want to check out some dubstep mixes.

Is it possible to narrow down what you're looking for at all?

'Weird' is mostly in the eye of the beholder, and hip-hop/rap/r&b-ish covers a wide range of music, especially when you throw in Tricky.

That said, if you like Pre-Millennial Tension, stuff like the Tricky/Grease/Muggs album, Gorillaz and The Streets might also be up your alley.

Boom Bip, the aforementioned Anticon label's output especially Buck65, Anti-pop consortium, stuff on the Def Jux label, I really love Aesop Rock but that also covers El-P, Cannibal Ox etc.
