Office technology survey questionaire

Where can I find sample questions and answers to create a technology survey of computer software and hardware in use in my office? The goal is to have office workers complete the survey on line using surveymonkey.com or a similar tool.
I am an attorney who recently joined a medium sized law firm with four offices. The technology (sw + hw) in use is very out of date. I want to effect some change and help create a strategic IT plan. To do so I would like to survey the employees about all aspects of their use of technology at the firm. Does anyone know of such a survey form already available for this purpose that can be adapted to online survey use on a site like surveymonkey.com?
