Making my own sheet music--how to add in chords?

I'm trying to insert graphic chord diagrams (little pictures that show where to put your fingers) into a text document that shows the chord names (like C-minor). Any chord name could match 4 or 5 possible diagrams. I'm trying to figure out a way, in any mac or web software, to give myself a little pop-up menu asking which of the four I'd like to insert--and then, when I choose it, insert it!
Can anyone point me in any direction for achieving something like this?
I'd ask the people who run Chordie how they do exactly that.

Response by poster: Great suggestions! I should have said, I'm looking for ukulele chords (didn't think it would make a difference). I'll have a look!

If you're looking for guitar, bass, mandolin, or ukulele headstock diagrams, like you'd find in a tab chart, you can get fonts that do what you want.
