Optimum Voice: How to get my fax machine to work with optimum voice?

I recently switched to Optimum Voice for my main phone line to save with the triple play feature. I only have one line and would like to get my fax machine to work as well.

My setup from what I can see is the following:
Wall Jack to Splitter
Splitter to Cable Modem
Splitter to Telephone

I'm confused as why a line goes to the cable modem at all, and why there is no line from the cable modem outward.

Where would I put the fax machine in this setup?

The line from the cable modem is going outward. It really doesn't need to be connected to the wall jack at all, unless you want your phone signal to go to the rest of the house. You should put the splitter in front of the cable modem and split the line to the telephone and fax.
