Custom notebook printing/binding services?

Looking for recommendations of printers... more details inside.

I am a designer and would like to design a notebook (for designers). Then, I'd like to get a small - medium quantity of them printed and bound with (probably) spiral binding (although possibly glued softcover).

Bonus points of the pages can be perforated for easy ripping out.

What are some good places that do that sort of thing?

Preferably cool hip companies with good prices, a website that works well, and some kind of proofing options.

well, you mentioned softcover, so I'll throw blurb.com in there. the drawback is you have to use their layout software, so if you're doing a lot of type, it might get weird.
