Forms needed

I need some examples of short employee self-evaluation forms.

Our small non-profit is currently using one that is seven pages long. When I asked if we could shorten it, they put me in charge of coming up with a new one. Sigh.

NAVPERS 1616/26 [2 page pdf] would have to be customized a bit to make sense for your organization, but could be a starting point.

Best answer: Check out the forms at UC Berkeley. The employee self-assessment is optional (1 page). The first question in it refers to the 'job success factors' listed in the main performance evaluation form (4 pages) for each job type.
