My Grandmother has dementia. She calls her old phone number 20+ times a day to ask for a ride home.

How do presidents call other presidents? With last week's phone call between President Obama and Iranian President Hassan Rouhani, it got me wondering how do heads of state call other heads of state? I imagine that one executive suite's switchboard calls the other's, but what's the diplomatic protocol for who picks up the phone first? Do they have super-secret phone numbers, or even phone numbers in the way we think of them? Are the calls private, or are is there an entire entourage listening in? Do they just hang up when they're finished, or is their an entire protocol for ending the conversation? What are some of the other "hidden" aspects of executive diplomatic communication and behind-the-scenes goings-on are there that the rest of us commoners never see?

I just moved to a new part of the state, and had a landline telephone installed through my local telco. I asked if they could port my old number (123)-xxx-xxxx to my new landline, and they said my old number was not in the same "Rate Center". I confirmed that they were telling the truth based on this AskMeFi post
I have the option to enable call forwarding from my old number to my new landline, which takes care of incoming calls.
Now I am turning to you, the hivemind, to find out how to dial out from my new landline, and have my old number, (123)-xxx-xxxx, show up on other peoples' caller ID.
I've done some searching and found a bunch of convoluted VoIP setups that would achieve this solution for me, but I'm not that technical, and I don't want to have to dial a million numbers just to make a single phone call from my land-line.
Does the hive mind have any simple solutions?

Require a replacement for my landline and the 30 bucks a month that it costs me as I now have 4 cell phones in the house.
Require a telephone device that (a) rings on more than one handset (b) has access to cheap long distance (c) good quality and dependable.
I realize Google Voice does a lot of this, but I'm in Canada and we don't have Google Voice available here yet or any indication of how soon it could be arriving.
Is all of this possible without a landline?
What services can you recommend to get me closer to my goal?

No dial tone inside my house, but the DSL is working. When I plug a phone directly into the grey box (a.k.a. Network Interface Device) on the outside wall of the house, at the same time disconnecting the link from the phone company's incoming line to the wiring inside the house, I get a dial tone! It seems that the phone company is still providing service. The evidence is exactly what I'd expect if there were a phone off the hook inside the house... but there isn't. Is there a cheap or free way to setup a phone number in Trinidad and Tobago and have it forward to an American number? Skype and Grand Central don't have numbers in that area code (868). A dozen other vaguely similar services don't, either. [more inside]

For the last month or two I've been hearing noises from the flat above me. Specifically, I hear a phone number being dialled (as if by a modem), followed by the standard telephone voice saying that the number dialled has not been recognised. This voice repeats itself for a while, before the whole process starts again. This goes on for hours at a time. What's going on?
