Once or twice a week at least, I hear a subway announcement that the line is running with delays "due to a disabled train." This might signify that the train is five minutes later than you expected, or two hours later. Does anyone know exactly what "a disabled train" means?

I'm trying to compile a list of subway or light rail systems that are open all night. So far all I have is the New York subway and the Chicago El, but there's got to be more than that. There's a lot of cities in the world!

I'm relatively new to New York and ride the subway every day. When I'm on the above-ground lines I notice another "fourth" rail in between the two running rails that doesn't look like it is used, but is securely tied down. Sometimes there's even a fifth rail, also in the middle. Usually this rail doesn't run continuously and it occasionally curves or has other oddities. What is this fourth rail for?
