How can I stop repeating certain behavioral patterns?

"We've had this conversation before. I don't want to have it again." I'm finding myself on the receiving end of this line way too often, from both my spouse and my employer. The problem is, I keep repeating bad patterns like forgetting to turn off the light as I leave the room at home, or not managing my project time effectively at work.
