I have now heard several managers utter the buzzword "Best Practices" when describing why they do the things they do. What is the source of this new buzzword, and why is it so prevalent?

I have a question regarding unethical hiring practices. This question has a few permutations to it. The main one is it looks like a woman will be interviewing for a position, and one person on the three-person interview panel is a man with whom she is having an affair. The other folks who will be interviewed for the job are aware of this. What is their recourse? What is the smartest course of action? [more inside]
While recognizing that I'd like your advice on the principles and best practices of a shared child custody agreement. I currently share both joint and physical custody with my daughter's mother, and things are going just fine, but wondering if you know of any resources (online, written, or your own experiences) to help me consider what is best for my daughter now and in the future, and maybe it can serve as a general resource for other parents' looking to establish positive custody agreements. Need advice about best method of circulating a petition for a ballot initiative in California. There is a ballot initiative in California to replace the death penalty with life in prison. I'm going with a friend to a shopping mall this Sunday to try and get signatures (they still need about 100,000). I've never done anything like this before. I need advice about the best way to go about it. [more inside]
I improved a website for a business. The client was satisfied throughout the project and at launch 1 yr ago. But he fell out with his partners and they're threatening to sue me. I billed about $5k for the site. The site now does everything he wanted - consistent navigation, improved graphics, data driven pages (it had been over 100 html pages with little consistency among them). Can I be sued for violating some best practices like not saving Includes in their own directory, having some HTML tags inside a SQL database, leaving test code and unused files on the production site and not optimizing queries, commenting-out unused code instead of removing it? Can I be sued for leaving the site vulnerable to SQL Injection attacks even if there were no actual damages? [more inside]

Is there anything one can do to fight a medical bill that appears to be incorrect? I visited the emergency room last fall. I paid a $100 copay at the time of service that appears to have never been applied to my bill. Despite several calls to the ostensible hospital billing office (really a cube-farm in Texas) pointing out some discrepancies, they've left the clock ticking, sent threatening notices, and are apparently determined to slash my credit rating if I don't pay *now*. What can I do? [more inside]
